Orrington Plaza (Chase Bank) Building
1603 Orrington Ave Suite 600
Evanston IL 60201
After entering Orrington Plaza through any of the surrounding doors, head to the security guard station (centrally located on the main level). You will need to check in with them to request access to the 6th floor suite. They will only require your first name and last initial. Only the elevators on your left hand side go to the 6th floor. Security will swipe your access up from that elevator bank.
There is a staffed reception desk on the 6th floor servicing the hours of 9am-5pm. If they are present at the time of your arrival, they can direct you to the waiting lounge. If no one is present, you are welcome to walk straight back toward the windows and have a seat in the waiting lounge. I will come there to meet you on your first visit.
For all follow-up visits, please wait in the waiting lounge and I will text you a “thumbs up” when you can come back to my office. Please wait for the “thumbs up” text message to come back to my office.

The first session will be both conversational and diagnostic. This session is an opportunity for both of us to become acquainted with our respective personality and communication styles, as well as to provide me with a solid diagnostic picture of your mental health journey up until now.
This is why I recommend a 90 minute intake session.
Diagnoses do not define you, but they are essential to the treatment planning process, as well as to determine if I am the best provider to oversee your continuing outpatient care.
I will take the lead in this session from what you indicate as important on your initial intake paperwork, and will assess any other clinical areas that may appear relevant to me.
My full intake process typically takes 2-3 sessions.

Although I am qualified to therapeutically support you through gender-dysphoric exploration or through the therapeutic processing of pursuing or completing any gender-affirming changes, I am not trained in providing gender-affirming care letters. I am happy to connect you with resources and providers who can.

I do not provide letters for approval of emotional support animals. I will direct you to any medical professional overseeing your care to acquire this documentation.

I do not complete paperwork for FMLA or STD (short-term disability) paperwork, but will collaborate with the physician or psychiatrist completing this paperwork on your behalf, at my administrative rate.
